1. Information we collect

Depending on how you interact with TESSIA, we may collect:

• Contact information, such as your email address.
• Care Continuity Check responses, including your answers about care coordination, continuity, backup, access to information, and the care situation you choose to describe.
• Open-text responses that you voluntarily provide in the Check.
• Email preference information, including whether you chose to receive occasional emails from TESSIA.
• Referral and campaign information, such as UTM parameters that help us understand which content or campaign brought you to the Check.
• Basic technical or submission information that may be processed by our website, form, hosting, or technology providers, such as timestamps, browser/device information, IP address, and approximate country or location information.

2. How we use your information

We may use the information we collect to:

• generate and send the personalized result or resource you requested;
• operate, maintain, troubleshoot, and improve TESSIA's tools and experiences;
• understand patterns in care coordination and evaluate whether our tools are useful;
• measure the performance of content and acquisition experiments;
• respond to questions or requests you send us;
• protect the security and integrity of our services; and
• comply with applicable legal obligations.

We aim to collect only information that is reasonably useful for these purposes.

3. Email communications

If you provide your email address to receive a personalized result, we may use that address to deliver the result and related transactional information.

We will send occasional marketing, educational, product, or update emails from TESSIA only when you have indicated that you would like to receive them. You can unsubscribe from those emails at any time using the unsubscribe method included in the message or by contacting us.

4. How we share information

We do not sell or rent the personal information you submit to TESSIA.

We may share or allow access to information with service providers that help us operate TESSIA, such as:

• form and survey providers, including Tally;
• cloud productivity, storage, document, spreadsheet, and email providers, including Google Workspace services;
• website hosting and infrastructure providers; and
• other vendors we may use to operate, secure, analyze, or deliver our services.

These providers may process information on our behalf under their own terms and privacy practices. We may also disclose information when reasonably necessary to comply with law, protect rights or safety, investigate misuse, or respond to a lawful request.

5. Data retention

We retain personal information only for as long as reasonably necessary for the purposes described in this Policy, including delivering requested resources, conducting research and product validation, maintaining appropriate records, resolving issues, and meeting legal obligations.

Retention periods may vary depending on the type of information and why it was collected. You may contact us to request deletion of information associated with you.

6. Data security

We use reasonable administrative and technical measures appropriate to the nature of the information we handle. However, no online service, storage system, or method of transmission can be guaranteed to be completely secure.

For that reason, please do not submit passwords, financial account numbers, medical record numbers, Social Security numbers, or other highly sensitive credentials through TESSIA forms.

7. Your choices and privacy requests

Depending on where you live, you may have rights regarding your personal information. You may contact us to request access to, correction of, or deletion of information associated with you, or to ask a question about how your information is used.

You may also withdraw your permission to receive TESSIA marketing emails at any time. Withdrawing from marketing emails does not prevent us from sending a resource or transactional message that you specifically requested.

To make a privacy request, email hello@tessiacare.com. We may need to verify enough information to reasonably identify the record connected to your request.

8. Browser signals and third-party tracking

Browser “Do Not Track” signals are not currently used to change how the TESSIA website operates. TESSIA does not currently use the personal information you submit through the Care Continuity Check for cross-site behavioral advertising.

Our hosting, form, and other technology providers may receive standard technical information when you use their services. Their handling of that information is governed by their own privacy practices.

9. Children's privacy

TESSIA's services are intended for adults managing or participating in care. We do not knowingly collect personal information directly from children under 13. If you believe a child has submitted personal information to us, please contact us so we can review and, where appropriate, delete it.

10. International processing

TESSIA and its service providers may process or store information in countries other than the country where you live. Data protection rules may differ between jurisdictions.

11. Changes to this Privacy Policy

We may update this Privacy Policy as TESSIA evolves or as our practices change. If we make material changes, we will post the revised Policy on this page and update the effective date above. When appropriate, we may provide additional notice through the website or by email.
